HOW TO BECOME A PATIENT
PAIN PATIENTS APPOINTMENTS
To be accepted as a patient you must be pre-screened in advance over the phone. Please call 561-330-3010
We don't take walk-in patients; most of our patients are referrals from other patients and from doctors.
Pain patients are required to send or Fax documentation of their chronic pain such as MRI reports, referrals, orthopedic, surgical, or neurologist reports and proof of prior or current medications such as pharmacy records.

The doctor will personally review each submission for approval.
Urine toxicology screens will be performed on all new pain patients.
All accepted patients must sign a Pain Contract with Dr. Sabates in which they agree for him to be to be the sole provider of pain medications and to follow all the rules and regulations of the Center.
DETOX PATIENTS APPOINTMENTS
Patients requesting opiate Detox. with the FDA approved SUBOXONE outpatient program must be also pre-screened on the phone.
The patient will be instructed to be opiate free at least twelve hours before Suboxone induction can be started. These patients require no specific medical documentation but also will require urine screens.
Relatives or significant others are encouraged to accompany the patient in order to help them understand and follow the addiction treatment protocol.
Mental health counseling will be offered but is not mandatory.
